Claims Department Overview
For covered services provided by Scott & White Health Plan network providers, members do not have to file a claim. The participating provider will file the claims on your behalf. Just make sure you present your ID Card at the time of service which identifies you as a member.
However; If you pay out-of-pocket for covered services you should file your receipt with pertinent information directly to the Health Plan at the below address for consideration of your claim. There is no claim form available to fill out.

Once the Health Plan receives your claim, you will receive an acknowledgement letter within 15 days. The acknowledgment letter will indicate if any additional information is needed. Claims are paid within 5 business days following notification of acceptance.
